What to bring to your appointment

  1. Form DS-11
  2. Both parents must be present at time of the appointment for children Age 15 and under. DS-3053 to be completed by absent parent
  3. Certified Birth Certificate (Long Form) or Certificate of Naturalization
  4. Two (2) 2X2 Photographs
  5. Valid ID w/ Photo:
    • U.S. Drivers License or Identification Card
    • USA/Visa
    • U.S. Passport
    • Consular I.D.
    • USA or Mexican Passport
  6. Money Order or Check for “Passport Fee” payable to U.S. Department of State
  7. Cash, Money Order or Check for “Execution Fees” made be payable to City of McAllen
